Summary

Ascend contracts with Tennessee Registered Nurses across the state.  Independent contractors administer assessments on individuals who have had a medical level of care denial for long-term care services and for whom an appeal has been requested.  These assessments are instrumental as they will give Ascend the needed information to make a level of care determination for use by Bureau authorities. 

 

We are looking for quality driven, person-centered professionals who possess the passion, experience, and skills to administer these assessments. 

 

This is contract work and clinicians are compensated for each completed assessment. 

Essential Duties

Administer on-site assessments for individuals with medical conditions based upon review of records, completing a comprehensive functional assessment, including direct and in-depth observation of Activities of Daily Living (ADLs) and conducting an interview with the individual and family member and/or significant caregiver.    

                                                               

Administer and submit completed assessments following Federal HIPAA regulations and Ascend's HIPAA related requirements, procedures, and policies.

 

Complete on-site assessments within the respective timeline specified by Ascend

Education and Experience Requirements

A Tennessee Registered Nurse with a minimum of two (2) years of experience providing medical care and services to individuals.

 

Home Health experience isn’t required but is a plus.
